Accident & Near Miss Form

Faculty of Engineering and Natural Sciences has a system to report, record, and investigate work-related accidents or near misses.

In case of an accident/near miss, Laboratory Safety Specialist or related Laboratory Specialist must be notified immediately. Laboratory Safety Specialist reports the accident/near miss to the SU Occ. Safety Specialist and Workplace Doctor via the Accident Form/Near Miss Form.

Report Form

* The incident that occurs in the workplace or due to execution of work, and that causes death or mental or physical disability of the integrity of the body.

** An incident where an accident didn't actually happen and which didn't cause any damage
